REGISTRATION DIRECTIONS
Follow the three easy steps to complete your registration for the 2011-2012 season.
Skaters may not get on the ice until registration is complete. Scholarships available.
1. Register with USA Hockey on-line www.usahockey.com.
The fee is $35.
6 & under is free (birth year 2005 & up), but you still need to register.
Print the receipt.
Or use e-mail option and forward to Kim Duggar, Registrar @ homrdugr@alaska.net . Record your number before you exit.
2. Print and complete forms from link below:
Consent to Treat
Participant Code of Conduct
3. Bring the following forms to the rink office and place in Registrar Box:
USAH Registration Receipt
Completed Consent to Treat
Completed Participant Code of Conduct
Payment OR Receipt PAY ON-LINE USING DONATE BUTTON TO LEFT
Copy of birth certificate if under 18 and a new member (MicroBells excluded)
FEE SCHEDULE:
MicroBells: $125 Jan – Mar. GEAR PROVIDED
Meets Sunday afternoons for one hour.
Mites – Adult: $150 ($125 before Oct. 1st) Monthly ice bills to follow.
$75 for members joining after January 2012
Family Discount:
1st and 2nd player full fee
3rd player 50% discount @ $75 ($62.50 before Oct. 1)
4th player 75% discount @ 37.50 ($31.25 before Oct. 1)
Discount does not apply to MicroBells.
