Registration

Please follow the three easy steps to complete your registration for the ’10-’11 hockey season.

1) Register with USA Hockey on-line. Go to www.usahockey.com and on the right side of the home page you will see register on line. Click and follow the directions. The fee is $35 (no charge for 6 and under, but still must register). Print the receipt. If you do not have a printer, there is an option to e-mail the receipt to Kim Duggar, HHA Registrar @ homrdugr@alaska.net .

2) Print the CONSENT TO TREAT and PARTICIPANT CODE OF CONDUCT on this website and complete.

3) Bring the three forms and payment to the rink or mail to the address below

Completed Participant Code of Conduct
and a copy of birth certificate if under 18 years old

FEE SCHEDULE AS FOLLOWS:

MicroBells: $125 for Oct - Dec or $50/month This includes gear and Saturday ice time.

All other levels Youth - Adult: $150 (monthly ice fees to follow which vary between groups)

Family Discount: First two members full fee, third member 50% @ $75, 4th member 75% @ $37.50

Family discount does not include MicroBells.
